Sitomet 50/1000 Tablet(Sitagliptin + Metformin Hydrochloride)
- Type 2 Diabetes Mellitus (T2DM)
- Used as an adjunct to diet and exercise to improve glycemic control.
- May be prescribed alone or with other antidiabetic agents.
- Take with meals to reduce stomach upset.
- Less likely to cause weight gain compared to some other diabetes drugs.
- Inform your doctor if you plan surgery under anesthesia.
- Report immediately if you develop deep/rapid breathing, persistent nausea/vomiting, or abdominal pain → possible lactic acidosis.
- Avoid excessive alcohol intake.
- Stay on your prescribed diet and exercise program for best results.
Adults
- Initial (not on Metformin): 50 mg Sitagliptin + 500 mg Metformin twice daily.
- Already on Metformin: Continue the current metformin dose and add Sitagliptin 50 mg twice daily (100 mg/day).
- Typical adjustment: 50 mg Sitagliptin + 1000 mg Metformin twice daily if tolerated.
- Maximum daily dose: Sitagliptin 100 mg + Metformin 2000 mg.
Extended-release (XR):
Start with Sitagliptin 50–100 mg + Metformin XR 500–1000 mg once daily (preferably in the evening with food).
Gradually increase if needed.
Max: Sitagliptin 100 mg + Metformin XR 2000 mg daily.
Pediatrics
<18 years: Safety and efficacy not established.
Renal Dose Adjustment
Check eGFR before initiation.
- eGFR <30 mL/min/1.73m² → Contraindicated.
- eGFR 30–45 → Do not initiate; if already on therapy, reassess risks/benefits.
- eGFR >45 → Monitor annually; adjust as required.
Take with food to minimize stomach upset.
Swallow whole with water; do not chew, crush, or break.
Take at the same time each day for best result
