Acemetacin is indicated for the treatment of various conditions including low back pain, osteoarthritis, postoperative inflammation, postoperative pain, and rheumatoid arthritis.
Acemetacin should be taken with food, preferably with or immediately after meals.
Monitoring parameters include blood pressure, complete blood count with differential during prolonged use, renal function tests, liver function tests, and observation for gastrointestinal effects and bleeding. Ophthalmologic exams may also be necessary during prolonged use.
In case of an overdose, symptomatic treatment is recommended. Measures such as administration of activated charcoal or gastric lavage within 1 hour of ingestion may be considered. Renal and hepatic function should be closely monitored, and symptomatic relief should be provided as needed.
Acemetacin may interact with other medications, increasing the risk of adverse effects. It may also alter the metabolism or excretion of various drugs, including anticoagulants, diuretics, cardiac glycosides, phenytoin, lithium, methotrexate, and others.
Acemetacin is a glycolic acid ester of indomethacin. Its precise mechanism of action is not fully understood, but it is believed to exert its pharmacological activity through inhibition of prostaglandin synthesis. Unlike other NSAIDs, it is considered a weak inhibitor of prostaglandin synthetase.
Acemetacin is well absorbed from the gastrointestinal tract, with 100% bioavailability after 10 days of therapy. It reaches peak plasma concentration approximately 2.6 hours after immediate-release administration and 6 hours after extended-release administration. It is metabolized in the liver to its major metabolite, indometacin, and eliminated via feces and urine, with an elimination half-life of approximately 1.1-1.8 hours.
Patients with various medical conditions such as bronchial asthma, chronic respiratory disease, hypertension, heart failure, gastrointestinal disorders, psychiatric disorders, and others should use acemetacin with caution. Concomitant use with other NSAIDs should be avoided. Extended-release capsules are not recommended for initial relief of acute pain. Mild to moderate renal and hepatic impairment, as well as elderly or debilitated patients, require special consideration. Pregnancy (first and second trimester) and lactation are also precautionary situations.
