Pharmacology

Sodium Picosulfate, a stimulant laxative from the triarylmethane group, is locally activated in the colon by bacterial enzymes. It stimulates colonic nerves, enhances peristalsis, promotes water and electrolyte retention in the lumen, and facilitates stool evacuation. This effect also helps restore the natural urge to defecate.

Dosage & Administration

Oral use only. Best taken at bedtime to induce a bowel movement by morning. Start with the lowest effective dose and adjust as needed. Do not exceed the maximum daily dose.

10 mg Tablets

Adults & children >10 yrs: 5–10 mg/day

Children 4–10 yrs: 2.5–5 mg/day

Oral Solution (5 mg/5 ml)

Adults & children >10 yrs: 5–10 ml/day (1–2 tsp)

Children 4–10 yrs: 2.5–5 ml/day (½–1 tsp)

Children 1 month–4 yrs: 0.25 ml/kg/day

Concentrated Solution (7.5 mg/ml)

Adults & children >10 yrs: 0.67–1.33 ml/day

Children 4–10 yrs: 0.33–0.67 ml/day

Drug Interactions

Diuretics and corticosteroids may increase the risk of electrolyte imbalance when used with high doses

Antibiotics may reduce the laxative effectiveness of Pikos

Contraindications

Intestinal obstruction or ileus

Acute abdominal pain with nausea/vomiting

Inflammatory bowel diseases

Severe dehydration

Known hypersensitivity to sodium picosulfate

Side Effects

Allergic reactions, dizziness, syncope

Gastrointestinal: abdominal pain, cramps, nausea, vomiting, diarrhea

Use in Pregnancy & Lactation

No known harmful effects in pregnancy, but avoid during the first trimester

Not excreted in breast milk in clinically relevant amounts

Precautions

Avoid prolonged use to prevent electrolyte disturbances and dehydration

Use cautiously in alcoholics, children, pregnant or breastfeeding women, and patients with liver disease or epilepsy

Overdose

Chronic overdose may lead to persistent diarrhea, electrolyte imbalances, metabolic disturbances, and kidney damage.
