Indications of Dopamine-Rotex Injection

A medication called dopamine-Rotex is used to treat low blood pressure brought on by a heart attack, an infection, heart surgery, or trauma. Heart failure can also benefit from it. By improving blood flow throughout the body and enhancing the heart’s ability to pump blood, it raises blood pressure. Additionally, Dopamine-Rotex increases urine production and may assist critically ill patients to avoid kidney failure.

Pharmacology

Dopamine stimulates dopaminergic receptors at low doses, causing renal and mesenteric vasodilation; at higher doses, it stimulates both dopaminergic and 1-adrenergic receptors, causing cardiac stimulation and renal vasodilation; and at high doses, it stimulates -adrenergic receptors, causing cardiac stimulation and renal vasodilation; large doses stimulate -adrenergic receptors.

Dosage & Administration

Intravenous-
Acute heart failure:

Adult: Initially, 2-5 mcg/kg/min, increased gradually by up to 5-10 mcg/kg/min according to the patient’s cardiac and urine output. Seriously ill patient: Up to 20-50 mcg/kg/min may be required.

Interaction of Injection

The arrhythmogenic impact of dopamine may be enhanced by cyclopropane and halogenated hydrocarbon anesthetics. -adrenergic agents counteract cardiac effects (e.g. propranolol, metoprolol). The vasoconstricting effect of high-dose dopamine may be counteracted by -adrenergic-blocking drugs. MAOIs have a longer-lasting and stronger effect. With phenytoin, there is a risk of hypotension and bradycardia. Hydrochlorothiazide or furosemide’s diuretic impact may be amplified. TCAs and guanethidine have a stronger vasopressor impact. Ergot alkaloids may cause significant vasoconstriction.

 

Contraindications

Pheochromocytoma, atrial or ventricular tachyarrhythmias, ventricular fibrillation, hyperthyroidism, and concurrent usage with ergotamine are all contraindications to dopamine administration.

Side Effects of Injection

  • Flushing (sense of warmth in the face, ears, neck, and trunk)
  • Headache
  • Increased heart rate
  • Palpitations

Pregnancy & Lactation

It is unknown whether dopamine passes the placental barrier during pregnancy. The advantages of using this product must be balanced against the potential hazards to the fetus.

Lactation: It is not advised for moms who are breastfeeding unless the expected advantages outweigh the potential dangers.

Precautions & Warnings

Active myocardial ischemia or recent MI in a patient with CV disease, cardiac arrhythmias, and/or occlusive vascular disease. Prior to treatment, correct hypovolemia. Lactation and pregnancy.
