Description

Tiginor (atorvastatin) selectively inhibits the enzyme HMG-CoA reductase, which plays a crucial role in cholesterol production. Blocking this enzyme lowers cholesterol synthesis in the liver and increases LDL receptor activity, enhancing LDL clearance from the bloodstream.

  • Absorption: Rapidly absorbed, peaking within 1–2 hours. Bioavailability of the active drug is ~14%, while ethe nzyme inhibition effect is ~30%.
  • Distribution: Broadly distributed (volume ~381L); ~98% bound to plasma proteins; limited entry into red blood cells.
  • Metabolism: Extensively metabolized by CYP3A4 into active metabolites that significantly contribute to its therapeutic effect.
  • Excretion: Mainly eliminated via bile; less than 2% excreted in urine. The drug’s half-life is about 14 hours, but enzyme inhibition lasts 20–30 hours.

Tiginor is prescribed alongside dietary measures to lower elevated levels of total cholesterol, LDL cholesterol, apolipoprotein B (Apo-B), and triglycerides in cases where diet and non-drug interventions are insufficient. Its uses include:

  • Lowering total and LDL cholesterol in patients with heterozygous or homozygous familial hypercholesterolemia.
  • Managing high cholesterol and triglycerides in mixed dyslipidemia (Fredrickson types IIa and IIb).
  • Treating elevated triglyceride levels in hypertriglyceridemia (Fredrickson type IV).
  • Treating dysbetalipoproteinemia (Fredrickson type III).
  • Reducing ischemic heart events in patients with coronary artery disease who have raised LDL cholesterol, whether symptomatic or asymptomatic.
  • Lowering cholesterol in patients with hypercholesterolemia related to diabetes or post-renal transplantation.
