Ramipril is an angiotensin-converting enzyme (ACE) inhibitor that is converted into its active form, ramiprilat, after administration. It works by blocking the conversion of angiotensin I into angiotensin II, a potent vasoconstrictor.
This action leads to:
- Reduced levels of angiotensin II
- Decreased vasoconstriction
- Lower aldosterone secretion
As a result, blood pressure decreases, and cardiovascular protection is achieved. Ramipril is also effective in heart failure management and long-term prevention of cardiovascular events. It is well tolerated and suitable for prolonged therapy.
Ramipril is prescribed in the following conditions:
- Management of hypertension, either alone or in combination with other antihypertensive agents
- Treatment of congestive heart failure, often alongside diuretics
- Management of patients showing clinical signs of heart failure within the first few days after an acute myocardial infarction
- Treatment of diabetic and non-diabetic nephropathy (both early and established stages)
- Reduction of the risk of heart attack, stroke, and cardiovascular death in high-risk individuals, including those with:
- Coronary artery disease
- History of stroke
- Peripheral vascular disease
- Diabetes mellitus with additional risk factors (e.g., hypertension, abnormal cholesterol levels, smoking, or microalbuminuria)
