Generic Name: Valsartan 160mg + Hydrochlorothiazide 25mg
Manufacturer: Incepta Pharmaceuticals Ltd.
Indications
A pair of medications are combined in Valsartil Plus 160/25. This combination is indicated for the treatment of hypertension.
Pharmacology
Angiotensin receptor blockers are a class of medications that include the oral pill valsartan (ARBs). It is a specific and dynamic angiotensin II antagonist that is working on the AT1 subtype orally. Vasoconstriction is a result of angiotensin’s interaction with the receptors, which causes the blood vessels to constrict, increasing blood volume (hypertension). Valsartan inhibits angiotensin II receptor activity. Valsartan widens blood arteries and reduces blood weight without altering heart rate by inhibiting the action of angiotensin. Valsartan has a substantially stronger preference (20,000 times stronger) for the AT1 receptor than the AT2 receptor. It does not bind to or fragment other hormone receptors or particle channels that are known to be crucial for controlling heart rate.
Hydrochlorothiazide is a kind of thiazide diuretic. Thiazides affect the electrolyte reabsorption mechanisms in the renal tubules, causing a nearly equal increase in salt and chloride excretion. The diuretic action of hydrochlorothiazide decreases plasma volume, which increases plasma renin activity, aldosterone secretion, urinary potassium loss, and serum potassium levels. Because renin-aldosterone communication is mediated by angiotensin II, co-administration of an ACE inhibitor tends to reverse the potassium loss brought on by these diuretics.
Dosage & Administration
- Hypertension: If Valsartan and Hydrochlorothiazide alone are unable to control a patient’s blood pressure, they should be combined with valsartan once daily. When used with 25 mg of hydrochlorothiazide, the maximum recommended dose of valsartan is 320 mg.
- Elderly Individuals: Elderly patients don’t require an initial dosage adjustment.
- Use in children: Its safety and efficacy in young patients are unknown.
Interaction
- Diuretics with Valsartan: After commencing Valsartan medicine, patients taking diuretics may notice an unusual reduction in blood pressure. No clinically relevant drug interactions involving thiazide diuretics have been found.
There are more and more agents around. Potassium supplements or potassium substitutes containing salt: Because Valsartan decreases the synthesis of aldosterone, potassium supplements or potassium substitutes containing salt can result in hyperkalemia.
Lithium Salts: As with other drugs that eliminate salt, lithium clearance may be reduced. Serum lithium levels should be closely watched if lithium salts are given.
Digoxin and Warfarin are two other medications that interact. - Hydrochlorothiazide: Alcohol, barbiturates, and narcotics may make thiazide diuretics’ ability to lower blood pressure worse, leading to orthostatic hypotension.
Insulin and other oral diabetes medications: The dosage of these medications may need to be changed.
Synergistic effects are produced by additional antihypertensive drugs.
Colestipol resins and cholesterol: Anionic exchange resins interfere with hydrochlorothiazide absorption.
Contraindications
The combination of Valsartan and Hydrochlorothiazide is contraindicated in people who are hypersensitive to either component of this medication. Due to the Hydrochlorothiazide component, this drug is contraindicated in patients who have anuria or hypersensitivity to other medications derived from sulfonamides.
Side Effect
The Valsartan and Hydrochlorothiazide combination is generally well tolerated, with few adverse effects.Â
- Nausea
- Taste change
- Diarrhea
- Upset stomach
- Impaired renal function
- Dizziness
- Decreased blood pressure
- Headache
- Abdominal pain
- Constipation
- Fatigue/ Lethargy
- Muscle cramps
- Increased blood lipid level
- Electrolyte imbalance
- Increased blood uric acid
- Glucose Intolerance,
Pregnancy & Lactation
- Valsartan should not be used during pregnancy since it can harm or even kill the fetus in the second and third trimesters. Valsartan should be terminated as soon as possible if pregnancy is discovered.
- Mothers who are breastfeeding: Valsartan is not known to be excreted in human milk. Breast milk contains hydrochlorothiazide.
Precautions & Warnings
- The drug Valsartil Plus 160/25 may dehydrate you. Be sure to stay hydrated and let your doctor know if you have severe thirst, a dry mouth, or muscle weakness.
- Individuals with mild to severe hepatic impairment, especially those with biliary obstructive disease, should exercise caution because the majority of Valsartan is eliminated in the bile.
- Thiazides may cause azotemia, so patients with pre-existing renal impairment may need to reduce their dosage or stop taking it altogether.
- Patients with heart failure and those who have experienced a myocardial infarction should exercise caution before initiating treatment.
