Indications

A medication called Ursocol 150 is used to dissolve some types of gallstones, stop them from forming, and treat primary biliary cirrhosis, a type of liver disease. It is a bile acid that aids in the breakdown of cholesterol that has crystallized into gallbladder stones.

Pharmacology

A naturally occurring bile acid called ursodeoxycholic acid is used to treat a number of hepatobiliary conditions. The action of ursodeoxycholic acid is based on a decrease in bile cholesterol synthesis. By lowering dietary cholesterol absorption, reducing hepatic cholesterol production, and not impairing bile acid synthesis, ursodeoxycholic acid achieves this.

Bile’s composition shifts from supersaturated to unsaturated as a result. Because it encourages the growth of liquid cholesterol crystal complexes, ursodeoxycholic acid also facilitates the movement of cholesterol from the gallbladder into the intestine for excretion.

  • Cholangiocytes are shielded from the cytotoxicity of hydrophobic bile acid by ursodeoxycholic acid, which aids in the treatment of cholestatic liver disease.

Ursodeoxycholic acid is completely absorbed in the upper gut. Between 30 and 150 minutes pass before serum concentration reaches its maximum. The percentage of absorption varies from 60% to 80%. After absorption, ursodeoxycholic acid travels to the portal vein where it is taken up by the liver, conjugated with an amino acid (glycine or taurine), and then released into the hepatic bile ducts. Ursodeoxycholic acid is present in very small amounts in the blood and is only slightly excreted in the urine. The biological half-life of ursodeoxycholic acid ranges from 3.5 to 5.8 days.

Dosage & Administration

  • Gallstone dissolution: 8–12 mg/kg/day, given either all at once at night or in divided doses.
  • 2-4 divided doses of 10-15 mg/kg/day for primary biliary cirrhosis
  • 600 mg daily for acute viral hepatitis.
  • 300 mg/day for alcoholic fatty liver.
  • 25–30 mg/kg/day for primary sclerosing cholangiocarcinoma
  • Gallstones and Non-Alcoholic Steato Hepatitis Dissolution: 13–15 mg/kg/day.

Interactions

Ursodeoxycholic Acid should not be used with medicines that raise bile cholesterol, such as oestrogenic hormones. Concurrent administration of Ursodeoxycholic acid with bile-acid binding medications such as antacids, charcoal, or cholestyramine should be avoided, since this may impair the efficacy of Ursodeoxycholic acid therapy.

Contraindications

Inflammatory intestinal illness, non-functioning gallbladder, calcified and pigmented gallstones

Side Effects

Commonly reported side effects are nausea, vomiting, diarrhea, gallstone opacification, and pruritus.

Pregnancy & Lactation

Category B pregnancy. In pregnancy, no indication of danger has been documented. It has been successfully used to treat cholestasis in pregnant women throughout the third trimester with no adverse effects. Breastfeeding problems have not been recorded in humans.

It should be used cautiously in those with liver disease.
