Indications

Trimetazidine hydrochloride is suitable for adults as add-on therapy for the symptomatic treatment of patients with stable angina with insufficient control or intolerance to first-line antianginal therapy.

 

Pharmacology

Trimetazidine dihydrochloride is the first inhibitor of ketoacyl-CoA thiolase (KAT). KAT is a metabolic anti-ischemic drug that has proven to be beneficial to all coronary artery patients. Trimetazidine dihydrochloride inhibits the fatty acid pathway by inhibiting the 3-ketoacyl CoA thiolase and transfers oxygen to the glucose pathway. Since the glucose pathway produces energy more efficiently, the same oxygen produces more energy, making the heart more active. In addition, the aerobic oxidation of glucose prevents the production of lactic acid, thereby preventing angina pectoris.

 

Dosage & Administration

The dose is 0ne tablet of 35 rng of Trimetazidine twice daily during meals. The llenefit of the treatrnent should be assessed after three months and Trimetazidine should be discontinued if there is no treatment response.

 

Interaction

Barbiturates, tricyclic antidepressants, and alcohol may impair the capacity of patients to metabolize high dosages of Napa. Overdosage of Napa might cause hepatotoxicity if consumed with alcohol. Anticonvulsants and oral steroid contraceptives cause an increase in liver enzymes, which can impede therapeutic Napa levels by increasing first-pass metabolism or clearance.

 

Contraindications

Patients who are hypersensitive to the active ingredient or any of the excipients should avoid using trimetazidine. It’s also not recommended for people who have Parkinson’s disease, parkinsonian symptoms, tremors, restless legs syndrome, or significant renal impairment.

 

Side Effect

Dizziness, headache, Abdomtnal pain, diarrhoea, dyspepsia. nausea and vomitin, Rash, pruritus, utlicaria, Asthenia.

 

Pregnancy & Lactation

There are no data on the use of trimetazidine in pregnant women. Animal studies have not shown direct or indirect harmful effects on reproductive toxicity. As a precaution, it is best to avoid using trimetazidine during pregnancy. Whether trimetazidine is excreted in human milk is unclear. The risk to the newborn/infant cannot be ruled out. Trimetazidine should not be used during breastfeeding.

 

Precautions & Warnings

Trimetazidine is not a curative treatment for angina attacks, nor is it the initial treatment for unstable angina. It is also not a treatment for myocardial infarction.
