Nitrin Sublingual Spray

Indications

Nitroglycerin sublingual spray is used to treat angina pectoris brought on by coronary artery disease for either prophylaxis or immediate relief.

Pharmacology

The primary pharmacological action of nitroglycerin is the unwinding of vascular smooth muscle, which has more obvious effects on veins and fringe supply routes while also having a vasodilator effect on both. Enlargement of the post-capillary vessels, including large veins, promotes blood pooling at the heart’s periphery and reduces venous return, which lowers the weight of the heart’s cleared-out ventricular end-diastole (pre-load). Reduced systemic vascular resistance and blood vessel weight are caused by artery unwinding (after-load).

Administration & Dosage

• One or two metered sprays should be applied under the tongue as soon as an attack starts.
• It is advised to use no more than three metered sprays in a 15-minute window.
• It is advised to seek immediate medical attention if the chest pain doesn’t go away.
• Spraying nitroglycerin may be used as a preventative measure five to ten minutes before engaging in activities that could cause an acute attack.

Interaction

When alcohol is combined with Nitroglycerin, it can cause severe hypotension and collapse. Oral Nitroglycerin may improve dihydroergotamine bioavailability. When calcium channel blockers, phenothiazines, and tricyclic antidepressants are taken together, orthostatic hypotension can occur.

Contraindications

Hypersensitivity to nitrates or any other formulation component. Hypertrophic obstructive cardiomyopathy causes hypotension, hypovolemia, severe anemia, cerebral haemorrhage and brain trauma, mitral stenosis, and angina. Phosphodiesterase inhibitors for the treatment of erectile dysfunction are given at the same time.

Side Effect

Headache, facial flushing, dizziness, nausea, vomiting, feelings of weakness, postural hypotension, and reflex tachycardia are all possible nitrate side effects.

Pregnancy & Lactation

Pregnancy Classification C. In pregnant women, there are no adequate and well-controlled studies. Nitroglycerin should only be administered to pregnant women if absolutely necessary. Whether nitroglycerin is excreted in human milk is unknown. Because many drugs are excreted in human milk, care should be taken when administering Nitroglycerin spray to a nursing mother.

Precautions & Warnings

• When using Nitroglycerin in the early stages of an acute myocardial infarction, it’s important to pay close attention to hemodynamics and clinical status.
• While being treated, it may result in headaches, which typically go away after a few days. If it continues, let your doctor know.
• With time, you might become tolerant to Nitrin Sublingual, which would make the next time you take the same dose less effective. To prevent this, adhere to the dosage schedule suggested.
• While receiving treatment with Nitrin Sublingual, refrain from taking drugs (prescribed for erectile dysfunction) like sildenafil or tadalafil as they may lower blood pressure.
• In patients with severely impaired renal or hepatic function, hypothyroidism, malnutrition, or hypothermia, nitroglycerin should be used with caution.
