Tildex 90 contains ticagrelor, an antiplatelet medicine commonly referred to as a blood thinner. It helps prevent harmful blood clots from forming inside blood vessels, thereby lowering the risk of serious cardiovascular events such as heart attack and stroke.
It is particularly beneficial for patients who have recently experienced a heart attack or severe chest pain related to heart disease (acute coronary syndrome), especially those who have undergone procedures like stent placement (PCI) or coronary artery bypass surgery. Tildex 90 is generally prescribed together with low-dose aspirin for better protection against clot formation.
The medicine may be taken with or without meals but should be taken regularly at the same time every day for optimal benefit. Treatment should not be stopped suddenly without consulting a doctor, as discontinuation may increase the risk of another heart attack or stroke.
Uses of Tildex 90

Dosage & Administration
- Take exactly as prescribed by the physician
- Swallow the tablet whole with water
- Do not crush, chew, or break the tablet
- Can be taken with or without food
Adult Dose
- Loading dose after acute coronary syndrome: 180 mg orally (two 90 mg tablets)
- Maintenance dose during first year after ACS: 90 mg twice daily
- After one year with history of myocardial infarction: 60 mg twice daily
- Usually given together with aspirin:
- Initial aspirin dose: 325 mg
- Maintenance aspirin dose: 75–100 mg daily
Aspirin maintenance dose should generally not exceed 100 mg/day.
Renal Impairment
No dose adjustment is usually required.
Hepatic Impairment
Contraindicated in moderate to severe liver disease.
Missed Dose
If a dose is missed, take it as soon as remembered. If it is close to the next scheduled dose, skip the missed dose and continue the regular dosing schedule. Do not double the dose.
Common Side Effects
- Bleeding
- Shortness of breath (dyspnea)

- Nausea
- Diarrhea
- Cough
- Fatigue
- Back pain
- Chest discomfort
- Hypotension
- Fainting
- Atrial fibrillation
Bleeding episodes are usually mild but may occasionally become serious.
Precautions & Warnings
Use cautiously in patients with:
- Increased bleeding risk
- Planned surgery or dental procedures
- Asthma or COPD
- Bradycardia risk
- Gout or hyperuricemia
Patients should inform healthcare providers before any surgical or dental treatment, as temporary discontinuation may be necessary.
Contraindications
Tildex 90 should not be used in patients with:
- Active bleeding (e.g., stomach ulcer, brain bleeding)
- History of intracranial hemorrhage
- Severe hepatic impairment
- Hypersensitivity to ticagrelor or any component of the formulation
Pregnancy & Lactation
Use during pregnancy and breastfeeding only if clearly needed and under medical supervision. It is not known whether ticagrelor passes into human breast milk.
Drug Interactions
Tildex 90 may interact with:
- NSAIDs and anticoagulants (increased bleeding risk)
- Ketoconazole, clarithromycin, ritonavir, atazanavir
- Rifampicin, dexamethasone, carbamazepine, phenytoin
- Digoxin
- Simvastatin doses above 40 mg
- Cyclosporine
Patients should inform their doctor about all medicines they are taking before starting therapy.
