L-Amlo is a calcium channel blocker used alone or together with other blood pressure medicines to treat hypertension (high blood pressure). By lowering blood pressure, it helps reduce the risk of serious cardiovascular complications such as stroke and heart attack.

Pharmacology

L-Amlo contains levamlodipine, the pharmacologically active isomer of amlodipine. It belongs to the dihydropyridine calcium channel blocker group.

It works by reducing the movement of calcium ions into vascular smooth muscle and cardiac muscle cells. Its effect is more pronounced on blood vessels, causing them to relax and widen, which lowers blood pressure and reduces the workload on the heart.

Levamlodipine reaches peak blood levels approximately 6–12 hours after oral administration, with an oral bioavailability of around 64–90%. Food does not significantly affect its absorption. It is extensively metabolized in the liver, primarily through the CYP3A4 pathway, and is mainly eliminated through the urine.

Dosage & Administration

L-Amlo should be taken orally.

  • Usual initial adult dose: 2.5 mg once daily
  • Maximum recommended dose: 5 mg once daily
  • Elderly patients or those with liver impairment: Treatment may be initiated with 1.25 mg once daily.
  • When adjusting the dose, a waiting period of approximately 7–14 days is generally recommended between dose changes, unless faster adjustment is clinically necessary and the patient can be monitored closely.
  • Children 6–17 years: The usual effective dose is 1.25–2.5 mg once daily. Doses above 2.5 mg daily have not been adequately studied in this age group.

Drug Interactions

Certain medicines may affect the action or blood levels of L-Amlo:

  • CYP3A inhibitors: Medicines that inhibit CYP3A may increase amlodipine exposure and may require dose adjustment.
  • CYP3A inducers: Their effect on amlodipine levels is not well established.
  • Simvastatin: Amlodipine can increase simvastatin exposure; the simvastatin dose should generally not exceed 20 mg daily when used together.
  • Cyclosporine or tacrolimus: Amlodipine may increase their blood concentrations, so regular monitoring may be required.

L-Amlo can be taken with or without food, as food does not significantly affect its absorption. Avoid grapefruit products and natural licorice unless your doctor advises otherwise.

Contraindications

L-Amlo should not be used by patients with a known allergy or hypersensitivity to amlodipine or its components.

Side Effects

Common side effects may include:

  • Difficulty breathing
  • Dizziness
  • Fast heartbeat (reflex tachycardia)
  • Low blood pressure
  • Abdominal pain
  • Swelling or edema

Less common effects may include:

  • Diarrhea
  • Back pain
  • Headache
  • Fatigue
  • Nausea

Pregnancy & Breastfeeding

Available information from post-marketing reports has not established a clear risk of major birth defects or miscarriage associated with amlodipine use during pregnancy. However, pregnant women should use L-Amlo only after consulting their doctor.

No harmful effects have been observed in breastfed infants exposed to amlodipine, but breastfeeding mothers should discuss its use with their healthcare provider.

Precautions & Warnings

Low Blood Pressure:
L-Amlo can occasionally cause symptomatic hypotension, particularly in people with severe aortic stenosis. Because its action develops gradually, sudden severe hypotension is less likely.

Angina or Heart Attack:
In patients with significant coronary artery disease, angina may occasionally worsen or a heart attack may occur after starting treatment or increasing the dose. Seek medical attention if chest pain becomes more frequent or severe.

Liver Impairment:
Amlodipine is extensively processed by the liver and remains in the body longer in patients with impaired liver function. Therefore, people with severe hepatic impairment should start with a low dose and have their treatment adjusted gradually under medical supervision.

Overdose

An excessive amount of L-Amlo may cause marked widening of blood vessels, significant low blood pressure, and possibly a rapid heartbeat. Suspected overdose requires prompt medical attention.
