Description

Aprima (Apremilast) is prescribed for adult patients in the following conditions:

  • Active Psoriatic Arthritis
  • Moderate to Severe Plaque Psoriasis in individuals eligible for systemic therapy or phototherapy
  1. Pediatric Use: Not recommended under 18 years of age due to lack of safety and efficacy data
  2. Pregnancy: Category C – use only if benefits clearly outweigh potential fetal risks
  3. Lactation: Unknown if excreted in human milk; caution advised while breastfeeding

Apremilast is an oral phosphodiesterase-4 (PDE4) inhibitor.

By blocking PDE4, it raises intracellular cAMP levels, leading to a reduction in proinflammatory cytokines such as:

  • TNF-α
  • IL-2
  • Interferon-γ
  • Leukotrienes
  • Nitric oxide synthase

This helps rebalance the immune system’s inflammatory response by modulating multiple cytokine pathways, rather than targeting a single one.
