Bisoprolol Fumarate is a highly selective ß1 adrenergic receptor blocker, displaying the greatest affinity for ß1 receptors compared to other beta-blockers. It selectively inhibits ß1 receptors in the heart and vascular smooth muscle, reducing heart rate and cardiac output, which lowers arterial blood pressure. Unlike non-selective ß-blockers, Bisoprolol does not adversely affect lipid metabolism or HDL-cholesterol levels during long-term use.

The pharmacokinetics of Bisoprolol allow for once-daily dosing, ensuring consistent plasma concentration levels with minimal variability.

• Absorption & Bioavailability: Over 90% of Bisoprolol is absorbed from the gastrointestinal tract, with an absolute bioavailability of 88% due to minimal first-pass metabolism (<10%). Food does not affect its absorption.
• Distribution: Bisoprolol is widely distributed, with a medium distribution volume of 3.5 L/kg.
• Metabolism: It undergoes oxidative metabolism without conjugation, and the resulting polar metabolites are excreted renally. The primary metabolic pathway involves CYP3A4 (~95%), with minor involvement of CYP2D6.
• Elimination: Bisoprolol clearance is balanced between renal excretion of the unchanged drug (~50%) and hepatic metabolism (~50%). The elimination half-life is 10-12 hours.
