Indications

It is effective for treating panic disorder, whether or not it includes agoraphobia. Unexpected panic attacks and worries about a recurrence, fear of impact, or the attack’s consequences are characteristics of panic disorder.

Dyskinesias, myoclonic seizures, and the minor variant of Lennox-Gastaut syndrome may also be treated with it alone or in conjunction with other medications. Patients with absence seizures (mild illnesses) who do not respond to succinimide may benefit from it.

In controlled clinical trials, the effectiveness of clonazepam for prolonged use—more than 9 weeks—has not been thoroughly investigated. Clinicians should periodically reassess the long-term efficacy of the drug for specific patients if they decide to use clonazepam for a protracted period of time

Pharmacology

Clonatril 0.5 mg Tablets have typical benzodiazepine pharmacological effects, such as anticonvulsant, sedative, muscle relaxing, and anxiolytic effects. The increased GABAergic neurotransmission at inhibitory synapses is the mechanism by which benzodiazepines exert their central effects. Positive allosteric regulation increases the affinity of GABA receptors for neurotransmitters in the presence of benzodiazepines, which enhances the effect of released GABA on postsynaptic transmembrane chloride flux.

There are also studies done on animals that demonstrate how clonazepam affects serotonin. Clonazepam can quickly inhibit a variety of paroxysmal activities, including spikes and wave discharges in absence seizures (small seizures), slow spikes, systemic spikes, and spikes over time, according to animal data and human EEG studies. or other places, or unpredictably shaped peaks and waves. Dosage & Administration

Oral:

Adults: 

• For adults with seizure disorders, the first dose shouldn’t be more than 1.5 mg/day divided into three doses. The dosage can be raised by 0.5 to 1 mg every three days until seizures are adequately under control or until side effects make a dosage increase impossible. Each patient’s maintenance dosage needs to be tailored to their response. The maximum dose that is suggested each day is 20 mg.
• Adults with panic disorder should start with a 0.25 mg dose, divided into two doses. After three days, the 1 mg/day target dose for the majority of patients may be raised.

Pediatric Patients: 

The initial dose for infants and children (up to 30 kg of body weight or 10 years of age) should be between 0.01 and 0.03 mg/kg/day but not to exceed 0.05 mg/kg/day given in two or three divided doses in order to reduce drowsiness.

Injection:

Adults: 

• By slow IV injection or IV infusions, administer 1 vial (1 mg). If necessary, repeat this dosage (1-4 mg are usually sufficient to reverse the status). Adults should not receive more than a total dose of 10 mg at a rate of injection that does not exceed 0.25 to 0.5 mg per minute (0.5 to 1.0 ml of the prepared solution).

Infants and children: 

• half of a vial (0.5 mg) by slow IV injection or by IV infusion.

Interaction

Phenytoin, carbamazepine, and phenobarbital pharmacokinetics do not appear to be affected by clonazepam. Clonazepam’s impact on the metabolism of other medications has not been studied.

Contraindications

Patients with a history of benzodiazepine hypersensitivity should not take it, nor should those who exhibit clinical or biochemical indicators of severe liver disease. It is not advised for acute narrow-angle glaucoma, but it can be used in open-angle glaucoma patients who are taking the appropriate medication.

Side Effects

The most frequent side effects of clonatril 0.5 mg Tablet are central nervous system depression (CNS). Patients receiving treatment for seizures have a reported drowsiness rate of about 50% and an ataxia rate of about 30%. In some cases, they might get better with time; about 25% of patients have been said to have behavioral problems. Some of the symptoms that may appear include aphonia, coma, tremors, vertigo, disorientation, sadness, amnesia, hallucinations, hysteria, increased libido, insomnia, psychosis, and palpitations.

Pregnancy & Lactation

Pregnancy: 

• Preclinical research does not rule out the possibility that Clonatril 0.5 mg Tablet may cause birth defects. There is evidence that anticonvulsants can be teratogens based on epidemiological analysis. However, it is challenging to identify which drug or drug combination is the cause of neonatal defects from published epidemiological reports. It’s also possible that other factors, like genetics or epilepsy itself, have a greater impact on birth defects than drugs do. In these situations, the drug should only be administered to expectant mothers if the potential advantages outweigh the risks to the fetus. When pregnant, Only use Clonatril 0.5 mg Tablet when there are strong indications. High doses taken in the final trimester of pregnancy or during delivery can result in poor diet, hypothermia, hypotonia, mild respiratory depression, and irregular heartbeats in the fetus. It should be kept in mind that seizures can be brought on by both pregnancy itself and abrupt drug withdrawal. In rare cases, newborns experience withdrawal symptoms from benzodiazepines.

Nursing Mothers: 

Mothers taking clonazepam shouldn’t breastfeed even though research has shown that very little of the drug’s active ingredient makes it into breast milk. Breastfeeding should be discontinued if clonazepam is clearly indicated.

Precautions & Warnings

In patients with multiple distinct seizure disorders, Clonatril 0.5 mg Tablet may increase the frequency or hasten the onset of generalized tonic-clonic seizures.

Anticonvulsants may need to be added or their dosage increased as a result of this. Valproic acid and Clonazepam combined can result in absent status.

Storage Conditions
