Indications of Don-A 15

  • The dyspeptic symptom complex includes the symptoms of gastroesophageal reflux disease, esophagitis, and delayed stomach emptying.
  • Upper abdominal discomfort, a feeling of abdominal fullness, and abdominal distension
  • Early satiety is characterized by eructation, flatulence, and early satiety.
  • Feeling queasy and sick
  • Stomach contents coming back up into the mouth, either with or without heartburn
  • Indigestion but no ulcers
  • Acute nausea and vomiting are caused by functional, organic, infectious, or dietary factors, as well as nausea and vomiting brought on by radiotherapy or pharmacological therapy, and nausea and vomiting brought on by migraine.
  • Parkinson’s disease-related nausea and vomiting brought on by dopamine agonists.
  • Increasing the transit time of barium in subsequent radiological tests

Pharmacology

Domperidone may be a dopamine antagonist that primarily targets the stomach’s and the Chemoreceptor Trigger Zone’s (CTZ) dopamine receptors. Its gastroprokinetic activity is based on the dopamine receptors it blocks, which affect gastrointestinal tract motility. Domperidone has almost no effect on the dopaminergic receptors in the brain because of its flimsy passage through the blood-brain barrier, thereby avoiding psychotropic and neurologic side effects. Domperidone improves antral and duodenal peristalsis, restores normal upper gastrointestinal tract motility and tone, and directs pylorus withdrawal. It also promotes gastric purging. Additionally, domperidone increases esophageal peristalsis and decreases the weight of the esophageal sphincter, preventing the release of gastric contents.

Dosage & Administration

Domperidone should be taken if necessary before going to bed and 15–30 minutes before meals.

The typical Domperidone oral dosage is as follows:

  • Adults: 10-20 mg every 6 to 8 hours (1-2 tablets or 10 to 20 ml suspension). Domperidone can be taken up to 80 mg per day.
  • Children: every 6 to 8 hours throughout the day, 2-4 ml of suspension per 10 kg of body weight or 0.4 to 0.8 ml of pediatric drops.

Distension of the stomach:

  • Adults: 10-20 mg every 6 to 8 hours (1-2 tablets or 10 to 20 ml suspension).
  • Children: every 6 to 8 hours throughout the day, 0.2 to 0.4 mg/kg (2-4 ml suspension/10 kg or 0.4 to 0.8 ml pediatric drops/10 kg) body weight.

In conditions that are acute or subacute (primarily acute nausea and vomiting):

  • Adults: every 6 to 8 hours, 20 mg (2 tablets or 20 ml suspension).
  • Children: every 6 to 8 hours throughout the day, 0.2 to 0.4 mg/kg (2-4 ml suspension/10 kg or 0.4 to 0.8 ml pediatric drops/10 kg) body weight. (The maximum treatment time for acute nausea and vomiting is 12 weeks.)

Using suppositories, by rectum

  • Adults (30 mg to 60 mg every 4 to 8 hours), including the elderly.
  • Children: For those weighing 10 to 25 kg, the daily maximum rectally administered dose is 30 mg. If necessary, the dose may be spread out throughout the day.

Twelve weeks is the maximum treatment 

Interaction of Don-A 15ml

Domperidone may lessen the hypoprolactemic effects of bromocriptine. Domperidone’s influence on Gl function may be countered by antimuscarinics and opiate analgesics. Exercise caution when combining domperidone with MAO (monoamine oxidase) 

Contraindications

Domperidone is not recommended for patients who have a known hypersensitivity to the medicine or for neonates. When gastrointestinal stimulation is potentially harmful, such as gastrointestinal hemorrhage, mechanical obstruction, or perforation, do not use domperidone. Patients with a prolactin-releasing pituitary tumor are also advised against taking it (prolactinoma).

Side Effects of Don-A 15ml

Since young children’s blood-brain barriers have not yet fully developed, there is a high risk of extrapyramidal effects when using domperidone on them. Domperidone is highly metabolized in the liver, so patients with hepatic impairment should use it with caution.

Pregnancy & Lactation

Because the safety of domperidone has not been established, it is not recommended for use during pregnancy. The teratogenic effect on the fetus has not been shown in animal research. Domperidone can help with galactorrhea and postpartum lactation. It’s secreted in breast milk, but in such small amounts that it’s not considered dangerous.

Precautions & Warnings

Domperidone should be used with extreme caution in children due to an elevated risk of extrapyramidal effects due to an incompletely established blood-brain barrier in young children. Domperidone should be taken with caution in patients with a hepatic impairment since it is highly metabolized in the liver.
