Indications

In the treatment of individuals with diabetes mellitus, a mixture of 75% insulin lispro protamine solution and 25% insulin lispro injection (rDNA origin) is used to control hyperglycemia.

In the treatment of individuals with diabetes mellitus, a 50 percent insulin lispro protamine suspension and 50 percent insulin lispro injection (rDNA origin) mixture is also advised for the control of hyperglycemia.

Pharmacology

Insulin lispro is a synthesized human insulin analogue with a short half-life. Insulin lispro protamine is a suspension of crystals made by mixing insulin lispro with protamine sulfate under the right conditions for crystal formation. They work together to keep glucose metabolism in check.

Dosage & Administration

Combination rapid-onset (faster than regular insulin) and intermediate-acting insulins in fixed dose. Dose regimen varies among patients depending on metabolic needs; typical daily insulin requirements range between 0.5-1 unit/kg

Interactions

Oral antidiabetic drugs, ACE inhibitors, disopyramide, fibrates, fluoxetine, MAOIs, propoxyphene, salicylates, somatostatin analogs (e.g., octreotide), and sulfonamide antibiotics can all increase the effects. Corticosteroids, niacin, danazol, diuretics, sympathomimetic drugs, isoniazid, phenothiazine derivatives, somatropin, thyroid hormones, oral contraceptives, and lithium can all reduce the effects. Beta-blockers and clonidine can conceal hypoglycemia symptoms.

Contraindications

Insulin Lispro Protamine & Insulin Lispro is contraindicated in patients who are hypersensitive to insulin lispro or any of the excipients in the formulation, as well as during bouts of hypoglycemia.

Side-Effects

Hypoglycaemia; hypokalemia, oedema; pruritus; pulpitation, nausea, rash; hypersensitivity reactions; lipoatropy or lipohypertrophy with SC Inj.

Pregnancy

Pregnancy Classification B. There are no appropriate and well-controlled studies in pregnant women OR animal reproduction research have failed to indicate a harm to the fetus. Although animal research have revealed a negative effect, adequate and well-controlled investigations in pregnant humans have found no risk to the fetus in any trimester.

Lactation: It is unknown whether this substance is dispersed in breast milk; it is compatible with breastfeeding, although lactating women may need to adjust their dosage; caution is advised.

Precautions

Renal or hepatic impairment; pregnancy and lactation; switching from another insulin Serum glucose, potassium, electrolytes, HbA1c, and lipid profile should all be monitored. Illnesses that occur at the same time, particularly infections.

Storage Conditions

Refrigerate between 2 and 8 degrees Celsius. Do not allow yourself to become frozen. If you don’t need to refrigerate insulin for immediate use, keep it in a cold place away from heat and light. Insulin can be stored at room temperature for up to a month.
