Indications

A drug called Lijenta 5 is used to treat type 2 diabetes mellitus. To control blood sugar levels, it is used in conjunction with a healthy diet and regular exercise. This assists in avoiding serious diabetes complications like kidney damage and blindness. When diet and exercise alone, or other medications, are insufficient to control your blood sugar level, Lijenta 5 is typically prescribed. It may be prescribed by your doctor alone or in combination with other diabetes medications.

Pharmacology

Linagliptin is recommended for people with type 2 diabetes mellitus to help with glycemic control. The incretin hormones GLP-1 (glucagon-like peptide-1) and GIP are degraded by the enzyme DPP-4, which is inhibited by linagliptin (glucose-dependent insulinotropic polypeptide). By stimulating the release of insulin from pancreatic beta () cells in a glucose-dependent manner and inhibiting the release of glucagon from pancreatic alpha () cells into circulation, linagliptin raises the concentrations of active incretin hormones.

Dosage & Administration

  • 5 mg of Lijenta once daily.
  • If linagliptin is taken along with metformin, the metformin dosage should be kept constant. 
  • A lower dose of sulfonylurea may be used in conjunction with another medication to lessen the risk of hypoglycemia.
  • Patients with renal impairment don’t need a dose adjustment.
  • Linagliptin may be taken at any time of the day, with or without food.

Interaction

Linagliptin does not inhibit other CYP isozymes but is a weak competitive and a weak to a moderate mechanism-based inhibitor of the CYP isozyme CYP3A4. Linagliptin poses a low risk for clinically significant interactions with other drugs, and clinical studies have shown that it has no clinically significant impact on the pharmacokinetics of metformin, glyburide, simvastatin, warfarin, digoxin, or oral contraceptives.

Contraindications

Hypersensitivity to the active substance or to any of the excipients.

Side Effects

  • Combining hypoglycemia (low blood sugar) with insulin or sulphonylurea
  • Nasopharyngitis (inflammation of the throat and nasal passages)
  • Cough
  • Headache
  • Allergic reaction

Pregnancy & Lactation

Pregnancy category B. There are no adequate and well-controlled studies on pregnant women. So, Linagliptin tablets should be used during pregnancy only if clearly needed. It is not known whether Linagliptin passes into breast milk or not.
