Glyceryl trinitrate, the active component in Nitrocard, enhances the production of nitric oxide, which in turn activates guanylate cyclase within vascular smooth muscle cells. This leads to smooth muscle relaxation, causing vasodilation. It lowers venous return (preload) and redistributes blood flow to ischemic areas of the heart, improving subendocardial perfusion. It also alleviates coronary artery spasms and dilates arterioles, thereby reducing afterload. These combined effects make it effective in relieving and preventing angina attacks.
