Olmenor Plus 5/40mg is a fixed-dose combination antihypertensive tablet containing Amlodipine Besilate 5mg (a dihydropyridine calcium channel blocker) and Olmesartan Medoxomil 40mg (an angiotensin II receptor blocker — ARB), manufactured by Drug International Ltd. — a leading pharmaceutical company in Bangladesh.
The two complementary mechanisms of action make Olmenor Plus significantly more effective than either drug alone at equivalent doses, with better tolerability (Amlodipine-associated oedema is reduced when combined with an ARB).
Indications (Uses)
- Hypertension (high blood pressure) in adults whose blood pressure is not adequately controlled on monotherapy
- Hypertension with concurrent type 2 diabetes (renal-protective benefits of ARB)
- Hypertension in patients at high cardiovascular risk (post-MI, chronic kidney disease)
- Reduction of risk of fatal and non-fatal cardiovascular events in hypertensive patients
Mechanism of Action (Pharmacology)
Amlodipine (CCB): Blocks L-type voltage-gated calcium channels in vascular smooth muscle and cardiac cells, causing arterial vasodilation and reducing peripheral vascular resistance — lowering systolic and diastolic blood pressure.
Olmesartan Medoxomil (ARB): Selectively blocks the AT1 angiotensin II receptor, preventing angiotensin II-mediated vasoconstriction, aldosterone secretion, and sodium retention. This independently lowers blood pressure and provides organ-protective effects.
Dosage & Administration
- Adults: 1 tablet (5/40mg) once daily, with or without food
- Standard starting dose: Amlodipine 5mg + Olmesartan 20mg once daily; titrate to 5/40mg after 1–2 weeks if needed
- Maximum dose: Amlodipine 10mg + Olmesartan 40mg once daily
- Take at the same time each day for consistent blood pressure control
Contraindications
- Pregnancy (2nd and 3rd trimesters) — Olmesartan can cause fetal renal dysfunction and death
- Concomitant use with aliskiren in patients with diabetes
- Severe hepatic impairment
- Known hypersensitivity to Amlodipine, Olmesartan, or dihydropyridines
Side Effects
Common: Peripheral oedema (ankle/foot swelling), dizziness, headache, flushing, fatigue.
Uncommon: Hypotension (especially on first dose or if dehydrated), palpitations.
Rare but serious: Sprue-like enteropathy (severe chronic diarrhoea with weight loss — discontinue and seek medical attention), angioedema.
Drug Interactions
- Potassium-sparing diuretics / potassium supplements: Risk of hyperkalaemia with ARBs
- NSAIDs (e.g., ibuprofen): May reduce antihypertensive effect and increase renal impairment risk
- Lithium: ARBs may increase serum lithium levels — monitor closely
- Dual RAAS blockade (ACE inhibitors + ARB): Not recommended — increased risk of hypotension, hyperkalaemia, and renal impairment
