Bilastine is a long-acting, selective H₁ receptor antagonist with high binding affinity—3x higher than Cetirizine and 5x higher than Fexofenadine. It has:
- No sedative effects
- No significant activity on muscarinic, serotonergic, dopaminergic, or other histamine receptor subtypes (H2, H3, H4)
- No hepatic metabolism
- Excretion: ~66% via feces, ~28% via urine
