Pmec Cream (not indicated for children under 2 years of age) is recommended as a second-line treatment for short-term and intermittent long-term management of mild to moderate atopic dermatitis in non-immunocompromised adults and children aged 2 years and above, who have not responded adequately to other topical prescription therapies, or when those treatments are unsuitable.
Pimecrolimus is a macrolactam derivative of ascomycin and belongs to the class of topical immunosuppressants. It works by inhibiting T-cell activation and reducing the release of inflammatory mediators from mast cells.
Although the exact mechanism of atopic dermatitis is not fully understood, studies show that:
- Pimecrolimus binds with high affinity to FKBP-12 (macrophilin-12).
- It blocks calcineurin activity, thereby suppressing transcription of early cytokines.
- It inhibits both Th1 (IL-2, IFN-γ) and Th2 (IL-4, IL-10) cytokine synthesis.
- It prevents antigen/IgE-induced release of pro-inflammatory mediators from mast cells.
