Indications

A type of antidepressant known as Repose 25 is a member of the SSRI (selective serotonin reuptake inhibitor) drug class. It is prescribed for depression and conditions connected to anxiety, such as panic disorder, obsessive-compulsive disorder, and post-traumatic stress disorder (PTSD). In addition, the premenstrual dysphoric disorder is treated with Repose 25. (depression and irritability before the menstrual period in women).

Pharmacology

Sertraline strongly and specifically inhibits 5-HT uptake by CNS neurons, which raises 5-HT concentrations at synaptic clefts and speeds up sustained 5-HT activity at postsynaptic receptor sites. It eventually causes depression to decline. Sertraline’s impact is further complicated by the fact that it slows down the brain’s serotonin turnover. It can only be taken once per day due to its prolonged elimination half-life.

Dosage & Administration

Adults-
Major depressive disorder:

  • Starting Dose: 50 mg
  • Therapeutic Range: 50-200 mg

Obsessive-compulsive disorder:

  • Starting Dose: 50 mg
  • Therapeutic Range: 50-200 mg

Panic disorder, Post-traumatic stress disorder, Social anxiety disorder:

  • Starting Dose: 25 mg
  • Therapeutic Range: 50-200 mg

Pediatric Patients (ages 6-12 years old)-
Obsessive-compulsive disorder:

  • Starting Dose: 25 mg
  • Therapeutic Range: 50-200 mg

The recommended interval between dose changes is one week.

Premenstrual dysphoric disorder (PMDD): 

  • Starting PMDD medication is 50 mg/day. Sertraline can be given continuously (every day during the menstrual cycle) or sporadically (starting the daily dosage 14 days prior to the anticipated onset of menstruation and continuing through the onset of menses). With each new cycle, intermittent dosing would be repeated.

Continuous: 

  • Patients not responding to a 50 mg dosage may benefit from dosage increases at 50 mg increments per menstrual cycle up to 150 mg per day.

Intermittent: 

  • Increasing the dosage up to a maximum of 100 mg per day during the following menstrual cycle may be beneficial for patients who are not responding to a 50 mg dose (and subsequent cycles).

Interaction

Given that sertraline is tightly bound to plasma protein and that other medications, such as warfarin and digitoxin, are also tightly bound to protein, its administration to a patient may result in a change in plasma concentrations, which could have unfavorable effects. On the other hand, Sertraline’s replacement by other medications that are tightly bound may have unfavorable effects. Sertraline can interact with a number of different drugs, including Cimetidine, CNS depressants like Diazepam, hypoglycemic drugs, and atenolol.

Contraindications

Sertraline is not recommended for individuals who have a known allergy to the medication or any of its excipients. It is not advised for people with moderate to severe hepatic impairment.

Side Effect

Sertraline can cause stomach trouble, diarrhea, constipation, vomiting, dry mouth, loss of appetite, weight gain, sleepiness, dizziness, headache, discomfort, burning or tingling in the hands or feet, excitation, and a sore throat, among other things.

Pregnancy & Lactation

Sertraline’s safety during human pregnancy has not been proved, despite the fact that no indication of teratogenicity has been found in animal research. Sertraline has been found to pass through breast milk. Its impact on breastfeeding infants has yet to be determined. If Sertraline therapy is required, it may be essential to stop breastfeeding.

Precautions & Warnings

  • You should exercise caution if you have allergies, seizures, heart disease, liver disease, or renal disease. 
  • This drug may make you feel sleepy or lightheaded. Driving or operating machinery are two examples of activities that call for attention and should be approached with caution. 
  • The elderly should use caution when taking this medication because they may be more sensitive to its side effects. 
  • Do not take this medication if you have recently used a monoamine oxidase inhibitor. 
  • It’s possible to experience suicidal ideation and behavioral changes (close monitoring of the patient after 2 to 3 weeks of use is required).
