Isotretinoin works by influencing cell cycle progression, differentiation, survival, and apoptosis, which collectively reduce sebum production. This action helps prevent clogged pores and limits the proliferation of acne-causing bacteria. Both isotretinoin and its metabolite, 4-oxo-isotretinoin, significantly decrease sebum production. Isotretinoin exhibits minimal binding to retinol-binding proteins (RBPs) and retinoic acid nuclear receptors (RARs), while tretinoin and 4-oxo-tretinoin selectively bind to the RAR-γ receptor, which may contribute to its anti-acne properties. By inducing apoptosis in sebocytes, it further reduces sebum levels. It also minimizes comedone formation by reducing hyperkeratinization through an unknown mechanism. Although isotretinoin does not directly kill bacteria, it creates an environment less favorable for bacterial growth by shrinking sebum ducts. Additionally, it may enhance immune responses and alter monocyte chemotaxis to reduce inflammation.
