Monotherapy / Combination Therapy: As an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus.
Limitations of Use:
Not indicated for type 1 diabetes mellitus or diabetic ketoacidosis.
Has not been studied in patients with a history of pancreatitis; the risk of pancreatitis in these patients is unknown.
Sitap (Sitagliptin)
Therapeutic Class: Dipeptidyl Peptidase-4 (DPP-4) Inhibitor
- Sitagliptin selectively inhibits DPP-4, the enzyme responsible for inactivating incretin hormones.
- Incretins such as GLP-1 and GIP stimulate glucose-dependent insulin release and reduce glucagon secretion.
- By increasing incretin levels, Sitagliptin enhances insulin secretion and suppresses hepatic glucose output in a glucose-dependent manner.
- Demonstrates high selectivity for DPP-4, with no inhibition of DPP-8 or DPP-9 at therapeutic doses.
Pediatric: Not established (<18 years).
Geriatric: Dose adjustment may be needed due to reduced renal function.
Renal Dose Adjustments:
Mild impairment (CrCl ≥50 mL/min): No adjustment.
Moderate impairment (CrCl 30–50 mL/min): 50 mg once daily.
Severe impairment / ESRD (CrCl <30 mL/min or on dialysis): 25 mg once daily.
