Telmisartan is a non-peptide angiotensin II receptor antagonist. Angiotensin II, formed from angiotensin I through the action of angiotensin-converting enzyme (ACE), is a key regulator in the renin-angiotensin system. It promotes vasoconstriction, stimulates aldosterone release, and influences cardiac and renal functions. Telmisartan works by selectively blocking angiotensin II from binding to the AT1 receptor in tissues like vascular smooth muscle and adrenal glands, reducing its vasoconstrictor and aldosterone-secreting effects.Telmisartan has a significantly higher affinity (over 3,000 times) for the AT1 receptor compared to the AT2 receptor. Since it does not inhibit ACE (kininase II), it does not influence bradykinin responses. Telmisartan also does not interact with other hormonal receptors or ion channels involved in cardiovascular regulation.
