Indications

  • Endocrine disorders include nonsuppurative thyroiditis, primary or secondary adrenocortical insufficiency, congenital adrenal hyperplasia, and hypercalcemia linked to cancer.
  • Pemphigus, bullous dermatitis herpetiformis, severe erythema multiforme, exfoliative dermatitis, mycosis fungoides, and severe psoriasis are examples of dermatological conditions.
  • Rheumatic diseases include post-traumatic osteoarthritis, juvenile rheumatoid arthritis, psoriatic arthritis, rheumatoid arthritis, ankylosing spondylitis, acute and subacute bursitis, acute nonspecific tenosynovitis, acute gouty arthritis.
  • Seasonal or persistent allergic rhinitis, bronchial asthma, contact dermatitis, atopic dermatitis, serum sickness, and adverse drug reactions are examples of allergic states.
  • Respiratory Disease: Aspiration pneumonitis, fulminating berylliosis, symptomatic sarcoidosis, etc.
  • Regional enteritis and ulcerative colitis are gastrointestinal conditions.
  • Edematous States: To cause diuresis or remission of proteinuria in nephrotic syndrome, without uremia, whether it be of the idiopathic type or that caused by lupus erythematosus. Hematologic disorders include acquired (autoimmune) hemolytic anemia, erythroblastopenia, secondary thrombocytopenia, and idiopathic thrombocytopenic purpura (RBC anemia).

Pharmacology

Prednisolone is a synthetic adrenocortical drug that functions similarly to glucocorticoids. The Phospholipase A2 enzyme, which produces inflammatory mediators like leukotrienes, SRS-A, and prostaglandins, is inhibited by prednisolone. Prednisolone is quickly and completely absorbed from the GI tract after oral ingestion. Prednisolone has a half-life of 2 to 4 hours before it is excreted and is 70–90% protein-bound in the plasma. The liver is where it is primarily processed, and urine is where it is eliminated.

Dosage and Administration

Adult-

Nephrotic Syndrome:

  • Once the urine has been protein-free for three days in a row (a maximum of 28 days), administer divided doses of 2 mg/kg/day (up to 80 mg/day) three to four times per day. Then, administer 1 to 1.5 mg/kg/dose every other day for four weeks.
  • Every other day for three to six months, administer a maintenance dose of 0.5 to 1 mg/kg.

Anti-inflammatory: 

  • 5 to 60 mg per day in divided doses 1 to 4 times/day.

Acute Asthma: 

  • 40-60 mg/day PO in a single daily dose or divided q12 hr for 3-10 days.

Allergic Conditions:

  • Day 1 dosages are as follows: 10 mg PO before breakfast, 5 mg after lunch and dinner, and 10 mg before bed.
  • On day two, take 5 mg orally before breakfast, after lunch, and after dinner, and 10 mg before going to bed.
  • Day 3: 5 mg PO should be taken in the morning, at noon, at dinner, and before bed.
  • Day 4: 5 mg PO should be taken before breakfast, after lunch, and before bed.
  • Day 5: Take 5 mg PO with breakfast and at night.
  • 5 mg PO on day 6 before breakfast.

Pediatric-

Asthma:

  • 1 year: Chronic: 10 mg taken orally every 12 hours. 10 mg taken orally every other day for maintenance.
  • 1-4 years old Chronic: 20 mg taken orally every 12 hours. 20 mg taken orally every other day for maintenance.
  • 5–12 years old: 30 mg orally, every 12 hours for acute. 30 mg taken orally every other day for maintenance.
  • 12 years: Chronic: 40 mg taken orally every 12 hours. 40 mg taken orally every other day for maintenance.
  • 0.05 to 2 mg/kg/day, split 1 to 4 times/day, are anti-inflammatory.
  • Immunosuppression: divided 0.05 to 2 mg/kg/day 1 to 4 times/day.

Interaction

Aminoglutethimide, Antacids, Barbiturates, Carbamazepine, Griseofulvin, Mitotane, Phenylbutazone, Phenytoin, Primidone, and Rifampin all decrease the effectiveness of prednisolone. The amount of potassium in the blood is decreased by prednisolone. If hypokalemia occurs, digitalis may cause cardiac arrhythmias. Care must be taken when administering vaccinations.

Contraindications

Unless specific anti-infective therapy is used, systemic infections can occur. Intolerance to any ingredient Herpes simplex ocular because of potential perforation

Pregnancy & Lactation

This medicine is not recommended for use during pregnancy unless considered essential by your doctor. It should only be used if the expected benefit to the mother is greater than any possible risk to the fetus. Corticosteroids appear in breast milk and could suppress growth, interfere with endogenous corticosteroid production or cause other unwanted effects.

Precautions & Warnings

Diabetes, hypertension, psychological disorders, osteoporosis, postmenopausal women, pregnancy, and chronic nephritis all require vigilance. Cushing’s syndrome, hyperglycemia, muscle weakness, increased susceptibility to infection, delayed wound healing, and psychological disorders are all possible side effects of long-term Prednisolone use.
