Folinex contains calcium folinate hydrate, a calcium salt of folinic acid (5-formyl derivative of tetrahydrofolic acid). It is a metabolically active form of folic acid that serves as a cofactor in one-carbon transfer reactions essential for purine and pyrimidine biosynthesis in nucleic acids.
- Mechanism in Anemia:
- Folinic acid helps to counteract the impaired thymidylate synthesis seen in folic acid deficiency, which disrupts DNA synthesis and leads to the formation of megaloblasts, causing megaloblastic and macrocytic anemia.
- Antidote to Folic Acid Antagonists:
- Calcium folinate is a potent antidote for toxic effects induced by folic acid antagonists (e.g., methotrexate, trimethoprim, pyrimethamine). It rescues normal cells by entering them preferentially over tumor cells, leveraging differences in their membrane transport mechanisms.
