Indications

Fixal belongs to the class of medications known as antihistamines. It is used to treat allergies such as hay fever, conjunctivitis, and some skin reactions such as eczema, hives, and bite and sting reactions. Watery eyes, a runny nose, sneezing, and itching are also relieved. Fixal should be taken first thing in the morning on an empty stomach. The dose may differ depending on the reason you are taking it. Take it exactly as directed by your doctor.

Pharmacology

Fexofenadine Hydrochloride is an antihistamine that works by blocking peripheral H1 receptors. After oral administration, it is rapidly absorbed and reaches maximal plasma concentration in 2-3 hours. It doesn’t appear to be able to pass through the blood-brain barrier.

Dosage & administration

Tablet

Adults and children over the age of 12 Seasonal Allergic Rhinitis: 

  • Once a daily dose of 120 mg (Ritch® 120) or 180 mg (Ritch® 180). 60 mg (Ritch® 60) twice daily for chronic allergic rhinitis and 180 mg (Ritch® 180) once daily for urticaria In patients with impaired renal function, a starting dose of 60 mg once daily is recommended. 6 to 11-year-old children 30 mg twice daily for seasonal allergic rhinitis In patients with impaired renal function, a starting dose of 30

Suspension

Children aged 2 to 11 years with Seasonal Allergic Rhinitis: 

  • Twice daily, 30 mg or 5ml (1 teaspoonful). In pediatric patients with impaired renal function, a starting dose of 30 mg or 5 mL (1 teaspoonful) once daily is recommended. 30 mg or 5 mL (1 teaspoonful) twice daily for chronic idiopathic urticaria The recommended starting dose for pediatric patients with impaired renal function is 30 mg or 5 mL (1 teaspoonful) once daily. Children aged 6 months to under 2 years Idiopathic Chronic Urticaria: 15 mg or 2.5 mL (1/2 teaspoonful) twice daily The recommended starting dose for pediatric patients with impaired renal function is 15 mg or 2.5 ml (1/2 teaspoonful)

Interaction

When combined with Erythromycin or Ketoconazole, plasma concentrations of Fexofenadine increased. Fexofenadine had no effect on Erythromycin or Ketoconazole pharmacokinetics. Fexofenadine absorption is reduced by antacids containing aluminum and magnesium hydroxide. It is best to wait 2 hours between taking Fexofenadine and antacids containing aluminum 

Contraindications

Fexofenadine is contraindicated in patients with a known hypersensitivity to Fexofenadine or any of its ingredients.

Side effects

In general, fexofenadine is well tolerated. Headache, fatigue, dizziness or drowsiness, and nausea are the most common side effects. Rashes, urticaria, pruritus, and hypersensitivity reactions like angioedema, dyspnea, chest tightness, flushing, and systemic anaphylaxis have been reported in rare cases.

Pregnancy and lactation

Fexofenadine is pregnancy category B2. There are no adequate and well-controlled studies on pregnant women exposed to Fexofenadine alone or through the administration of Terfenadine. Fexofenadine should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is not known if Fexofenadine is excreted in human milk. Because many drugs are excreted in human milk, caution should be exercised when Fexofenadine is administered to a nursing woman.

Precautions and Warning

Studies in the elderly, patients with hepatic impairment, and patients with cardiac disease exposed to Fexofenadine through administration of Terfenadine showed no statistically significant differences in pharmacokinetic parameters for Fexofenadine when compares to those pharmacokinetic parameters in healthy individuals. There is only limited data on the elderly and really or hepatically impaired patients. Fexofenadine should be administered with care in these special groups. Fexofenadine causes drowsiness less than other antihistamines; therefore caution should be advised during driving or operating potentially dangerous machinery.
