Flucloxacillin, an isoxazolyl penicillin, is effective against Gram-positive bacteria including penicillinase-producing strains. It demonstrates:
- Resistance to hydrolysis by penicillinase.
- Stability in gastric acid, allowing oral administration.
- It inhibits bacterial cell wall synthesis by binding to transpeptidase enzymes, thereby blocking cross-linking of peptidoglycan. This weakens the bacterial cell wall, leading to cell lysis and death.
Its acyl side chain provides steric hindrance, preventing β-lactam ring breakdown by bacterial penicillinase.

- Fluclox DS is prescribed for the management of infections caused by Gram-positive organisms, particularly penicillinase-producing staphylococci.
- Skin & soft tissue infections: boils, abscesses, carbuncles, infected eczema, ulcers, acne, cellulitis, impetigo, infected wounds, infected burns, furunculosis, otitis media, and otitis externa.
- Respiratory tract infections: pneumonia, lung abscess, empyema, sinusitis, tonsillitis, pharyngitis, and quinsy.
- Other infections: osteomyelitis, endocarditis, enteritis, urinary tract infections, meningitis, and septicemia caused by flucloxacillin-sensitive organisms.
- Prophylaxis: indicated during major surgical procedures (e.g., cardiothoracic and orthopedic surgery) to prevent post-operative infections.
