Prebichol (Obeticholic Acid) is prescribed for Primary Biliary Cholangitis (PBC) in the following scenarios:
- In combination with ursodeoxycholic acid (UDCA) for adults who have not responded adequately to UDCA after at least one year of use.
- As a monotherapy for adults who cannot tolerate UDCA.
- Prebichol can be taken with or without food.
- If taken alongside bile-acid binding resins, the obeticholic acid dose should be separated by at least 4 hours.
Obeticholic acid is a Farnesoid X receptor (FXR) agonist. FXR regulates key genes involved in bile acid synthesis, lipid metabolism, and glucose control. By activating FXR, obeticholic acid reduces bile acid production in the liver and enhances bile acid transport, reducing liver exposure to bile acids.
