Mirogabalin binds selectively and potently to the α2δ subunits of voltage-gated calcium channels (VGCCs). This reduces calcium (Ca²⁺) influx and neurotransmission in the dorsal root ganglia (DRG), inhibiting the release of neurotransmitters in presynaptic neuron endings. This mechanism reduces neuronal hyperexcitability and alleviates pain.
