- NO activates sGC, which increases production of cyclic guanosine monophosphate (cGMP).

- In heart failure, impaired NO production and reduced sGC activity worsen myocardial and vascular function.
By directly stimulating sGC—both independently and alongside NO—Vericiguat raises intracellular cGMP levels, promoting smooth muscle relaxation and vasodilation, ultimately improving cardiovascular performance.
