Entecavir, a guanosine nucleoside analogue, competitively inhibits HBV DNA polymerase (reverse transcriptase, rt) by interfering with:
- Base priming
- Reverse transcription of the negative strand from pregenomic mRNA
- Positive-strand synthesis of HBV DNA
After phosphorylation by kinases, entecavir is incorporated into viral DNA, thereby inhibiting HBV polymerase and blocking viral replication.
Teviral is indicated for the treatment of chronic hepatitis B virus (HBV) infection in adults and pediatric patients 2 years of age and older with:
Evidence of active viral replication, and
Either persistent elevation in serum aminotransferases (ALT/AST) or histologically active liver disease.
Pediatric: Safety and efficacy established for ≥2 years; not recommended <2 years.
Geriatric: Limited data in patients ≥65 years; caution advised, monitor renal function.
Renal Impairment (dose adjustment):
- CrCl ≥50 mL/min: 0.5 mg every 24 hr
- CrCl 30–49 mL/min: 0.5 mg every 48 hr
- CrCl 10–29 mL/min: 0.5 mg every 72 hr
- CrCl <10 mL/min, Hemodialysis or CAPD: 0.5 mg every 7 days
